During your initial evaluation, you will meet one-on-one with one of our experienced, licensed physical therapists. We will take the time to listen to your concerns, understand your symptoms and goals, and perform a comprehensive evaluation to identify the root cause of your condition.
Based on your evaluation, your physical therapist will develop a personalized treatment plan tailored to your unique needs and lifestyle. Together, you'll establish meaningful goals and create a plan designed to reduce pain, restore function, and help you return to the activities that matter most to you. Every step of your care is centered around your progress, comfort, and long-term success.
What to Bring to Your Appointment
To help ensure a smooth check-in process, please bring the following items to your first visit:
A valid driver's license or other government-issued photo ID
Your current insurance card
A prescription or referral for physical therapy from your physician (if applicable)
If you are being seen through Direct Access, please bring your healthcare provider's contact information
Completed new patient intake forms

Completing your paperwork in advance allows your therapist to review your medical history and concerns before your evaluation, giving you more time to focus on your treatment and goals during your visit.

Because insurance benefits vary by plan, we recommend contacting your insurance provider before your first visit to verify your physical therapy coverage, including any deductibles, copayments, coinsurance, or visit limitations.
Self-Pay (Out-of-Network)
We also welcome patients who choose to pay out of pocket or who have insurance plans that are out of network with our clinic.

Follow-Up Visits: $190
